If you look closer there is actually some small amounts of redness around some of the lines, so could've been done a couple of days before the pic was taken. Either that or it's some kind of skin condition.

and the no redness is the opposite of what you can see that is around the entire thing.. swelling? not everyone swells from getting a tattoo, i don't think my wrist even went red actually (well the blood, but after that was wiped off the next day when i removed the seran wrap)
you can never underestimate how fucking stupid people are, so it doesn't surprise me if this tattoo is real.. that much is certain :P
