It's not that difficult to keep a tune going with one hand, especially on an electric guitar. If it was an acoustic, he would play the melody minus chords and shake with the right hand. Back at school, my music teacher would play the rhythm on the piano and the melody on a violin. It's not that big a deal for an accomplished player.
