If it's real, he already has the hole in his hand from something else, then he just fired a blank. The blood splatter on the floor at his feet after he fires is a very nice touch, as are the powder burns on his palm.

The gun firing looked pretty real for a low caliber pistol, and the hole through the hand was nice trick, though.
