Insurance gets involved and now you don't just pay a doc to sew your dick back on after your dog bites it off, now you pay your doc, pay his malpractice insurance, pay the insurance agent, pay the fucking execs, pay the fucking bribes to the government officials, etc.
Go to countries with no insurance, government run or otherwise, and you can a reasonable cost to get the service in a reasonable time and have it done right. There is no reason medical procedures need to cost tens or hundreds of thousands of dollars. There's no reason a hospital should be billing over a thousand a night. It's all fucking insurance.

a progressive income tax (without loop holes) is actually the better solution, because sales taxes hit people with little income, families, collage students, pensioner,... hardest, because they have to spend the largest part of their income for their living expenses. rich people save more and invest their money. in the last decades the percentage (of national income) of capital incomes grew and the working incomes fell (not only in america!). so in best solution here would be a higher taxation of capital and speculation too. [that goes for all western countries].

This is the second book of that guy that I've read. Good stuff. The God Delusion is a recent book of his that I read first. Very good. This one, The Selfish Gene wasn't as good for me. It has good parts and boring parts, but overall it's good. Difficult science written for the layman.
